Medicare wants primary care providers to be more engaged in proactively managing their patients with chronic conditions, between and outside of office visits, and have created and enacted several initiatives to encourage and achieve this. We know navigating Medicare rules and regulations is challenging, to say the least, and that their communications are often unclear and poorly disseminated, but a great opportunity for primary care providers does exist.
Unfortunately, even when aware of these opportunities, busy independent providers rarely have the time or ability to fully understand their multiple benefits or how to implement them. Large multispecialty groups and healthcare organizations have numerous complex issues to contend with, thereby moving these programs down or even off their priority list. It’s no wonder that adoption rates remain so low, even though Medicare has attempted to simplify and improve the process.

As experienced clinicians and healthcare executives, we realized this unfortunate situation several years ago, and that as a result, many Medicare patients remain underserved. We decided to spend the time and do the research to fully understand these opportunities, how practices can efficiently initiate and operate them, given all the complexities and compliance issues. We then determined how we could work collaboratively, to create a straightforward and profitable partnership, resulting in enormous benefits to not just the provider or healthcare organization, but also their patients.
